We are proud to provide a state-of-the-art facility for the highest quality dental care available. It is one of our top priorities to protect the well-being of our valued patients. For this reason, our office meets and surpasses all OSHA (Occupational Safety and Health Administration) and CDC (Center for Disease Control) standards. We are confident that you will feel right at home in our office as we welcome all patients as if they were family.


  If you wish to set up an appointment, please feel free to call our receptionist or stop in to reserve an appointment, kindly give us a 24 hours notice. This courtesy makes it possible for us to schedule another patient who desires your appointment time. Appointments that are not cancelled within 24 hrs./ missed entirely are subject to charge.

We will make every effort to be on time for our patients and ask that you extend the same courtesy to us. In the event that unexpectedly long procedure needs to be performed or an emergency patient must be seen, we will attempt to advise you as soon as possible if we anticipate a delay.

Please advise us if there is any change in your health, medications, address, telephone number, marital status, employment, or insurance coverage.

Emergency Care:

If you develop a problem requiring emergency treatment, please call us immediately so that we may arrange to see you. If the emergency arises at a time other than during regular office hours, please call the office so that our answering service may contact Dr. Simon. In case of emergency you may also contact Dr. Simon 408 7724724.


We firmly believe in the importance of regular preventive dental health care. Although the responsibility of returning for this treatment lies with you, we will call or send you a written reminder when it is time for you to return. When you receive this notice, please stop in or telephone us for an appointment. If you do not receive a call or a note from our office, please contact us to set up a visit.

Financial Policy

Every effort is being made to keep the cost of dental care under control. You can help by paying for treatment at the time of your visit. For restorative treatment involving multiple steps and restorative treatment involving multiple steps and laboratory fees, a commitment payment of one-half the total fees is requested before treatment begins. The balance is payable prior to placement or cementation to the dentures or restoration. If your treatment will require several visits, you may request an estimate of the proposed services so that you can make appropriate financial arrangements. Several options are available for your convenience:

Payment at the time of service is rendered by cash or check.

Outside financing is also available.

Professional budget plan: After an initial down payment, the balance may be divided into monthly installments.

We request payment in full by the time of completion of any treatment procedure such as crowns, fixed bridges, or complete or partial dentures


Visa, Mastercard, Discover, American Express. 


As courtesy to our patients, we will complete and file insurance forms relative to dental services, dental insurance does not usually cover 100% of the charges incurred. We will be happy to determine the benefits allowed under your policy. Whenever your insurance company requires a predetermination of benefits before work is started, we will submit treatment plan to them. With the exception of emergency care, any extensive treatment will be delayed until benefits are established. Please note that you must complete your portion of the insurance form and sign it where indicated. If you desire to file and monitor your own insurance forms, you may do so with the understanding that we will request payment in full at the time services rendered. During the course of treatment, you will receive statements on a monthly basis regardless of your insurance coverage. Patients will be expected to begin payment on their portion of the balance at the time treatment begins.

We accept most dental insurance incld. Delta Dental, Aetna, Cigna, Metlife, etc.

We also offer financing through Care Credit, Chase Credit upon approval with 90 days or more with no interest.


If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.
